Dental implants in Winchester

A missing tooth does not stay a single problem. The teeth either side lean into the gap, the bone under it thins, and what was one decision becomes several. An implant is the replacement that goes into the jaw rather than resting on top of it.

Training and credentials
  • Associate Fellow, American Academy of Implant Dentistry

Dr. Orr earned it in 2014, after completing a MAXI course in Atlanta.

  • A 300-hour implant course

The MAXI program behind that fellowship is 300 hours of implant training.

  • Comprehensive Conscious Sedation

Dr. Orr holds the Tennessee Board of Dentistry's permit, so implant surgery here can be done under sedation.

Why here

An implant replaces the root as well as the crown

Whether it is one tooth or a whole arch, the planning, the surgery and your new teeth happen in one building in Winchester, TN. Losing a tooth, or most of them, does not have to be the end of your smile. Dr. Orr plans and places your implants himself, in the same office where you have your checkups.

  • The planning

Placing one is surgery, and the part that decides how it goes is the planning: how much bone is there, where the nerve runs, what the bite will do to it over the years.

  • Associate Fellow

Dr. Orr became an Associate Fellow of the American Academy of Implant Dentistry in 2014 after a course devoted to planning and placing implants.

  • Sedation

Dr. Orr gives the sedation himself if you want it.

Implant, bridge or denture?

Three ways to replace a tooth, and the right one depends as much on the teeth around the gap as on the gap itself.

Implant, bridge and denture, compared

What it is
Implant A post in the jaw with a tooth on it
Bridge A false tooth held by crowns on the teeth either side
Denture A removable plate carrying one or more teeth
What it needs from the teeth around it
Implant Nothing
Bridge Two healthy teeth cut down to carry it
Denture Nothing, though it rests on the gum and any remaining teeth
How long it lasts
Implant Many years with the same care a tooth needs
Bridge Depends on the teeth carrying it
Denture Years, with relines as the ridge changes
Cost, relative
Implant Highest at the outset
Bridge In between
Denture Lowest at the outset
Best when
Implant The neighboring teeth are healthy and you want it to stay put
Bridge The teeth either side already need crowns
Denture Several teeth are gone, or surgery is not an option

One tooth, several, or all of them

Which one you need is a question about how many teeth are gone and how much bone is left, and it is answered at a consultation with a scan.

  1. After: the space filled with an implant crown, in a smile showing the upper teeth.

    A single tooth

One missing tooth, root and crown

The post goes where the root was, and the crown on top matches the teeth beside it.

    What is placed
One implant, one crown on top
    Start to finish
Mostly healing, for a time your dentist sets
    Usually for
A tooth lost to a fracture, decay or an extraction
  2. A gloved dentist holding a lower jaw model, a run of teeth along the ridge.

    Several teeth

A run of missing teeth in one area

Two or more posts carry a fixed bridge, so fewer implants than teeth.

    What is placed
Two or more implants carrying a fixed bridge between them
    Start to finish
Mostly healing, for a time your dentist sets
    Usually for
Teeth lost together, where a denture is not wanted
  3. The upper arch of a dental teaching model, a full row of teeth along the palate.

    A full arch

Every tooth in an upper or lower jaw

Posts carry one fixed arch, replacing a full jaw of teeth.

    What is placed
    Implants carrying one fixed arch
    Start to finish
Mostly healing, for a time your dentist sets
    Usually for
A failing full arch, or a denture someone is done with

Am I a candidate?

Bone, gum health and anything else in your mouth that needs treating first decide it. Smoking and some medical conditions change the answer, which is why the medical history comes before the plan.

Full-arch implant dentures, placed here

This is the biggest of the three options above: a full-arch implant denture replaces every tooth in an upper or lower jaw, supported by implants instead of resting on the gum.

Dr. Orr places them in this office. IV sedation is available for the surgery.

Implants are available from several brands, in solid zirconia or titanium. Which suits you is decided at a consultation with a scan.

From loose dentures to implants

Two years of dentures that would not stay in, and what eating, talking and laughing are like with implants.

Why did you decide to get this procedure?
The reason that I chose to go in for the procedures that Dr. Orr offers is because I had had my dentures for possibly two years, a year and a half.
What problems were you having with your dentures?
They had gotten to where at first they were great. Even if they were relying there at the last, they got to where they would not stay in. I used a tremendous amount of the Fixodent and all the, the products that's offered for that, and that was not very successful before I decided to have the procedure done, the dentures were very loose. I started out in the morning with the Fixodent and all the go stuff. It would be by mid-morning. I'd have to reglue by lunchtime. You'd have to reglue. You could be talking and my top dentures would just fall down, which was rather embarrassing.
How did it affect what you could eat?
When I had my dentures, before I had this procedure done, I had to watch what I ate, anything sticky. You might come out with teeth and all, you know, but I could not eat anything like boiled corn. I could not eat beef jerky. I could not eat peanut brittle things of that sort. After I've had the procedure done, there is nothing that I have not been able to eat that I have found yet, and I'm very satisfied with what I have had done.
How did they make you feel comfortable?
His nurse at the time I went to school with, so I felt very comfortable there with them. The whole, the whole atmosphere at Dr. Orr's is just calm, relaxing, very kind. I've always had a fear of going to the dentist. I've never felt that fear. With Dr. Orr, you go in there, he's, he explains everything so calmly. He works at your pace. You know, he, he takes his time doing things and he tells you what he's doing. He tells you what he's gonna do. He makes sure you're comfortable.
Would you recommend sedation?
If I had to recommend this to anyone, I would tell them to trust Dr. Orr on the decision of sedation that they receive, because it is rather painful if it's just numbed up. As long as you do what he tells you, it's gonna be a breeze and it's gonna pay off in the long run.
How has this changed your life?
There's no way I can describe the difference in my life from before and after. The implants make it much easier to eat. You're more confident in speaking, more confident in laughing and having a good time like you would have normally. This has been such a relief to me. It has made you feel like you can get out and be in public again.

What happens, in order

Most of an implant is waiting. The appointments are short and there are not many of them; what takes the time is bone healing around the post, and there is no way to hurry it.

A dentist pinning 3D scans of a patient's teeth to a light box and pointing to one of them.
  1. The consultation and the scan

We look at the gap, the bone around it and where the nerve runs, and decide whether an implant is the right answer at all. Nothing is placed on this visit.

  2. Grafting, if the bone needs it

Where a tooth has been gone a while the bone under it has thinned, and it is built back up before anything is placed. This adds healing time, and not everyone needs it.

  3. Placing the implant

The post goes into the jaw. This is the surgical appointment, and it is the one people ask about sedation for.

  4. The healing period

The bone grows onto the post. You leave with something to fill the gap in the meantime and you get on with your life.

  5. The crown, bridge or arch

Once it has taken, an impression or a scan, then the tooth itself, shaped and shaded to the ones beside it.

  6. Living with it

Brush and floss it like a tooth, see us like a tooth. It does not decay; the gum around it still can.

Available with sedation, from nitrous to IV.

The questions that come up first

What are dental implants?

A dental implant replaces the root of a tooth as well as the crown. A post is placed in the jaw and a crown goes on top, so the tooth is held the way a natural one is. An implant can replace a single tooth, several teeth, or a full arch. How implants compare with a bridge or a denture is set out in the pros and cons article.

Dental implants: pros and cons
What is All-on-4?

All-on-4 is Nobel Biocare's registered name for a full-arch treatment concept, a full arch of teeth held on four implants. We place full-arch implant dentures here in Winchester, TN, with sedation available, and how many implants a patient needs is decided at the consultation.

What is All-on-4?
Does getting an implant hurt?

The placement is done numb. The days after are more like an extraction than a surgery, sore for a few days. If the worry is the appointment rather than the healing, sedation is available for it, from nitrous through to IV.

How long does the whole thing take?

Most of it is waiting rather than appointments, and your dentist sets how long from your scan. The post has to knit into the bone before anything is built on it, and that is biology rather than scheduling. If grafting is needed first, that adds healing time. The appointments themselves are short and there are not many of them.

How long do dental implants last?

The American Association of Oral and Maxillofacial Surgeons says that, properly cared for, dental implants can last a lifetime. Care means brushing and flossing the implant as you would a tooth and seeing the dentist as you would for a tooth. The implant does not decay, but the gum around it can still get disease.

How much do dental implants cost?

It depends on how many teeth are being replaced, whether the bone needs building up first, and what the tooth on top is made of. Insurance for implants is usually partial and sometimes nothing. There is no useful single figure until someone has looked at your mouth. Call the office and we will go through yours.

Does insurance cover implants?

Usually in part, sometimes not at all, and it varies by plan. We will do our best to check yours before you commit to anything, but please bring your plan details, and the patient is ultimately responsible for the cost. Billing and insurance are on our patient information page.

What is the difference between an implant and a mini implant?

A mini implant is a narrower, one-piece post. Because it is smaller it can sometimes go where there is less bone and it is often used to help hold a lower denture steady rather than to carry a single tooth on its own. Which kind suits a particular gap is a question for a scan and a conversation.

Am I too old for an implant?

Age on its own is not the criterion. What decides it is the bone, the health of your gums, and your general health and medications, which is why the medical history comes before the plan.

Can a denture be fixed to implants?

Yes. Implants can hold a denture in place so it does not move when you eat or talk, which is a different thing from a denture that rests on the gum. If that is what you are weighing, the dentures page goes through the options side by side.

Dentures
Do you take new patients?

Yes. An implant consultation can be your first visit: the scan and the conversation come before any treatment. What to bring is on our patient information page.
